NJ Chapter 193 child study team evaluations and case management services
On Track in a registered NJ agency that contracts with various Board of Education in NJ to provide Child Study Team evaluations under Ch. 193 of NJ Special Education Code.

Referral

To submit a child study team referral your child must be a residence of NJ between the age of 5-18.

A 407-1 Form must be completed and filled out by the child's parent or guardian. Please download a 407-1 for here. (A MS Word version is available to download here).

The 407-1 form should be complete and dropped off or mailed to our office at 1300 W County Line Rd. Lakewood, NJ 08701

A copy of the childs birth certificate should be included

A copy of a proof of residence should be included (ie parent license, utility bill)

Please note a copy of the 407 form will not be accepted as the board of ed requires an original.​
The Process- What to Expect

Our office will process and submit your 407-1 form to the board of ed (boe) for approval
-
The boe will review and confirm we can proceed with the referral
-
An On Track case manager will contact you within 20 days of boe approval to set up an initial determination meeting.

At the initial determination meeting, the team will meet with parent and school to discuss concerns and need for testing
-
If testing is deemed recommended, the case manager will assign testing to the various child study team members
-
Testing will take place within a timely manner (up to 90 days from the initial determination meeting) in the school setting (we can accommodate time sensitive needs)

Parent will be invited to review testing in the school at an eligibility meeting

When found eligible, team, parent and school will determine special education school based services and/or make further private recommendations
